I got into a car wreck June 27, 2009 and ended up flipping a car. Doctor says i "separated" my shoulder?

Now the deal is.. I was in the front passenger seat and the car was unfortunately a convertible with the top down when we wrecked. The car landed on the passenger side and was rocking as if it were a rocking chair. Well i was very intoxicated and i guess thought i was superman and stuck out my arm and pushed against the ground so the car would not flip on over on to us. The car did not flip thank god. The doctor said it would heal up in like 6 to 9 weeks but its been several months now and there is still a knot on my shoulder of course but i expected that, but my shoulder blade is not right.. me just standing there with my arms down you can obviously see its sticking out. Any ideas on what may have happened?
